Care Planning & the keyworker system |
- This system is in place to provide clients with individual, specific and continuous support from one person.
- A key worker is allocated to the client on arrival as part of a preliminary assessment which identifies his/her individual needs. An initial care plan is prepared by the allocated key worker in consultation with the therapeutic team and client. This is updated regularly as treatment progresses.
- The Key Worker will always be aware of their clients work and progress and will check in with the client on a daily bases to ensure there are no problems that the treatment teams are unaware of.
- Clients meet with their key worker on a regular basis for one to one sessions that review progress and inform treatment and care plans
- Key workers act as a central point of contact for care managers, next of kin and others associated with the client
This also provides a link in partnership working with: - Client, key worker and programme teams
